Insurance Information

    We are in-network with the following insurance companies: BCBS, UHC/Optum, and Medicare. We are Out-of-Network providers for all other insurance companies. Clients are responsible for fees at the time of their appointment (including copays and fees applied to your deductible). For self-pay clients; we provide detailed receipts should you choose to submit a claim to your insurance company. These reports are available upon request.

    We are not in network with Medicaid.

    Cancellation Policy

    We require 24-hour notice of any cancellation or rescheduled appointments. Late cancellations will be charged the full fee for the scheduled session. If you are late to your appointment; your appointment may need to be rescheduled depending on if time allows for the full session. If your appointment is rescheduled, you will be subject to a missed appointment fee. We require that clients pay any outstanding balances prior to scheduling their next appointment.
